Dr. Jan J. Shook, 82, of Port Neches, Texas passed away Tuesday, May 5, 2020 at his home surrounded by his family. Jan was born April 9, 1938 in Houston, Texas to Jessie Earl Shook, Sr. and Effie Agnes Giroir Shook. He was raised in Port Arthur, Texas and graduated from Thomas Jefferson High School, Class of 1956. Jan was married to his sweetheart of 58 years, Gail Patin Shook who he met when she was in the eighth grade. Jan joined the Army right out of high school serving six months before he went into the Army Reserves. He continued his education at Lamar University studying chemistry and then attended the University of Houston Pharmacy School. Upon graduation, he worked for five years as a Pharmacist at Port Acres Pharmacy which he owned. At this time in his life, Jan decided to enter Medical School, he spent 3 years at the University of Texas-Medical Branch before serving his internship and residency at John Peter Smith Hospital in Fort Worth. In 1971, he started his Family Practice in Port Neches and practiced for 44 years before his retirement in 2015. During his career, he worked at St. Mary Hospital, The Medical Center of Southeast Texas, and Mid-Jefferson Hospital where he served as Chief of Staff several times over the years. Jan spent a decade of his career as the plant Doctor for Motiva and also served as the Local Health Authority for the City of Port Neches from 1998 to 2015. Jan received many awards and accolades thru the years including Small Business Person of the Year. Visitation for family and friends will be held Saturday, May 9, 2020 from 10:00 a.m. until 12:00 p.m. at Levingston Funeral Home in Port Neches with a graveside service to follow at Oak Bluff Memorial Park with Reverend Shane Baxter officiating. He was preceded in death by his parents, and his brother, Jessie Earl Shook, Jr. Surviving relatives include his wife, Gail Shook of Port Neches, Texas; daughters, Dana Hasting and husband Morris of Katy, Texas, Kristin Sebright and husband Mark of Pearland, Texas; son, Jan Shook, Jr. and wife Karen of Cape Canaveral, Florida; brother, Dean Shook and wife Sandy of Richmond, Texas; 6 grandchildren, Jarret Shook and wife Elena, Ashley Shook, Morgan Hasting, Davis Hasting, Matt Sebright, and Katie Sebright; great granddaughter, Emma Shook. The family requests in lieu of flowers memorial contributions be made to the Charity of Your Choice.

To Dr. Shook's family, He was our dr. for 25 yrs. On his last day of work, my husband's referral for a test before a procedure was lost. Even though it was towards the end of the day, Dr. Shook made sure the referral was there. During the time he was seeing children; one of our son's was admitted to the hospital on Christmas day. Once discharged, Dr Shook called the house to check on him. We were very blessed to have had him as our Dr. Your husband touched a lot of lives in the MId-Co area. Jan.Was a cherished friend. we went to school together. we played basketball together. we went in the army together. we both started to college in chemistry together and we've been friends out whole life he was a wonderful man.

Dear Gail, So sorry to learn about Jan's passing. He is at peace now. It was a pleasure to have Jan as a first cousin. As you well know he was a special person and I am so glad that you found each other, had a good marriage and raised a nice family. Dear Gail, I am so sorry to learn of Jan's passing. I know he lived a life of service and care toward others and he will be missed. I have loved you since we became friends in Jr. High School many years ago and my love goes out to you in the loss of your dear husband. It's not "good-bye"...it's just "see you later". I have missed Dr Shook since he retired. It broke my heart to have to go to a new doctor. I was one of his 1st patients he took care of all 6 of my children. If one of the kids got sick he made sure I had enough medicine for all of them, he never made me bring them all in. He was so caring and easy to talk to. I remember he would call me at home after hours to give test results and make sure we were all ok if ill. There just aren't doctors like him any more. My thoughts and prayers go out to his family. The world has lost a great man.

I have loved both Gail and Jan over a lifetime. I just got this news and trying to figure out a way to get there in the morning from Dallas. All you can say at a time like this is thank God shortened his suffering. He took care of so many others during his lifetime. We will all miss Jans quite sense of humor. I love you Gail

This was the man that saved my life when he had me go to Rehab for alcohol addiction. He also saved the life of a friend of mine when he recognized what was wrong with him when other doctors didn't know, surgery was done by Dr Shook & saved his life. This man has a special place in my heart & in Heaven.

Words can not express,the way I feel about Dr Shook.He was my father's Dr and mine again after moving back from Fort worth,some 15 years ago...Not just our Dr but a family friend.,some of the best advice given to me by Doc for my during father,I will never forget..,Didn't get a chance to go by and see him like I should have,I assumed he would be around for ever.,He made all feel like they where his only patient...Too Dana and the rest of the Shook family,so sorry for your loss..But how blessed to have such a wonderful man part of your lives for so long RIP Doc.see you on the other side some day..God bless your family...

I trained with Dr Shook at John Peter Smith hospital Ft Worth. Dr Shook was an excellent physician and his greatest virtue was that he would listen as much as necessary and spend the time and effort necessary to achieve to best possible outcome with each patient. Dr Shook was one of the dwindling number of old time physicians with up to date info and skills. At Peter Smith we worked 70-80 hours a week yet he made time to check on his patients a second time before going home each day. In addition to practicing medicine Jan and Gail raised 3 wonderful children. I'm sure we would have all like to have seen Dr Shook enjoy more years of retirement however his family and the practice of medicine were his pleasures so he enjoyed life along the way. Gail you have been wonderful helping raise the children, maintaining your business enterprise and supporting Jan. You were with him, for him and in the end took care of him. A great life together now enjoy those children and grandchildren.

God Bless you Gail and family. He was a great man and Doctor. I remember you both from T.J. and you in Hussers. l graduated in 1959 and Larry in 1957. I recall what a beautiful lady you are.
